At this practice personal information from each patient is collected for the purpose of providing accurate and  comprehensive health care services.  The provision of these services includes obtaining reports including pathology, radiology, psychology, electroencephalogaphy, nerve conduction studies and other related services.

The confidentiality of your personal information held at this practice is of paramount concern.  My staff are very much aware of the importance of patient confidentiality, therefore, any disclosure of your personal information would be for the purpose of providing health care services or where required by law.  In the event that a request is made for a personal medico legal report or reports from insurance companies or work cover groups, then a signed consent form by yourself will be necessary before any such report is forwarded.

Every effort will be made to ensure that your personal information collected at this practice is accurate, complete and correct.  This information will be further updated with ongoing reports from other physicians that you might see and at follow-up consultations.

If you wish to examine your personal information held at this practice please contact the office manager.  In some  exceptional circumstances access may be withheld.  In that event the doctor will be happy to discuss these issues at a further medical consultation..  In addition, I may request you to have a further consultation so that I can explain the personal information as accurately as possible.  At times it is necessary to explain the records in easy to understand language.

To ensure the security of personal information held in this practice the written records are protected, while those held on computer are password protected and only accessible by my authorised staff. 

This practice will only use the Medicare numbers collected from patients for the purpose of billing for medical services provided to each patient.  Similarly any other government identifying number will not be used by this practice.
